When a combination biologic and oral immunotherapy trial for peanut allergy required more than a single randomization event, the sponsor ran into a challenge: how do you keep a trial blinded when subjects pass through three separate randomization decisions, an initial treatment assignment, a conditional re-randomization, and independent food challenge sequencing, without any one of those decisions revealing information about the others?
A single randomization event wasn’t going to be enough. The sponsor needed an IRT or RTSM platform built to manage multiple independent randomization layers, including hypothetical treatment assignments for subjects who discontinued early, while maintaining full blinding, audit-ready traceability, and dependable performance across every site.
YPrime designed and validated an integrated randomization system supporting three independent layers: the initial double-blind treatment assignment, a conditional re-randomization at the maintenance milestone, and randomized challenge-day sequencing at every food challenge visit. The maintenance-phase logic also generated a hypothetical assignment for subjects who discontinued or did not meet the dosing milestone, so intent-to-treat analysis could proceed without ever revealing a subject’s actual study status.
